Health Care Proxy Information
A health care proxy is a legal document that allows someone to choose a person they trust to make medical decisions for them if they are unable to do so themselves. This could happen if they are very sick or hurt and can't communicate. The chosen person, called the proxy, will work with doctors to ensure that the person’s wishes about their care are followed. It’s important to talk with the proxy about what kind of treatment is wanted so that they can make the best choices when needed.
